Town of Lapel utility customers can enroll in automatic payments by downloading, printing, and completing the enrollment form below.

Protect Your Banking Information

Do not email or upload your completed form. It contains sensitive bank routing and account information. Completed forms must be personally delivered to Town Hall or placed in the Town Hall night deposit box.

How to Enroll

1. Download the Automatic Payment Enrollment Form.
2. Print and complete all requested information.
3. Personally sign the completed form.
4. Deliver it to Town Hall or place it in the Town Hall night deposit box.
